Make Your Voice Heard for Better Health!

The Alabama Department of Public Health needs your help! We’ve all heard the statistics before:

  • Overall, 54.3% of Americans have at least one chronic condition (hypertension, diabetes, cancer, arthritis, asthma, COPD, depression, and kidney disease).

  • That number jumps to 61.6% for Alabamians.  

  • ·Alabama has one of the highest death rates from stroke in the nation, frequently ranking in the top five.

  • Alabama is in the "Stroke Belt," a region with a 27-34% higher stroke mortality rate than the rest of the country.

But it doesn’t have to be that way. Our Alabama Department of Public Health works diligently to help make our state healthier. But they need to hear from you so they can better serve you and your highest priority needs. 


How can you do that? It’s easy! ADPH developed the 2026 Community Health Opinion Survey so that you can make your voices heard. You must be 18 years or older to answer the survey; it takes about 10 minutes and asks questions about:

  • Things in our communities that affect our heath

  • The most important diseases and conditions that affect our health

  • Things that make it hard for people in our communities to get health care

  • Where do you go when you are sick or need advice about your health care?


What if you don’t have a computer or internet access? Ask a friend or family member who does to lend a hand. Many public libraries have computers and internet available to the community. Birmingham Public Library, Avondale Library, Homewood Library, and Tuscaloosa Library are just a few of the many public libraries that have computers. For most, you will need a library or a photo ID.
